When Ambassador Lwaxana Troi falls into a coma, Counselor Deanna Troi must delve into her mother's hidden memories and confront a long-repressed tragedy to save her, risking her own sanity in the process.

The Enterprise hosts a delegation of Cairn, a telepathic species, with Lwaxana Troi assisting in their understanding of verbal communication. During this time, Lwaxana begins behaving erratically, culminating in a breakdown in Ten Forward. When Beverly Crusher can find no physiological cause, Deanna Troi and Captain Picard suspect the Cairn's telepathy may be involved, especially after Maques, a Cairn diplomat, mentions a "dark place" within Lwaxana's mind.

Lwaxana collapses and falls into a coma. Maques reveals that Lwaxana's mind appears to have retreated into itself. Hoping to save her, Deanna Troi, with Maques's help, enters her mother's mind. She finds a surreal landscape filled with barriers, including a menacing wolf and an image of Captain Picard demanding she stop the process. Troi resists these illusions, realizing they are constructs of her mother's psyche meant to keep her away.

Troi encounters an idealized version of her deceased father, who urges her to leave Lwaxana in peace. Troi resists the emotional pull and continues her search, eventually encountering Hedril, a young Cairn girl, who seems to represent a key to Lwaxana's trauma. Troi confronts Lwaxana within her own mind, pressing her to reveal a hidden tragedy.

Lwaxana eventually reveals that she and her husband had another daughter, Kestra, who died in an accident as a child. The guilt and grief over Kestra's death led Lwaxana to repress all memories of her, deleting the relevant entries from her journals. Troi encourages her mother to confront this grief and forgive herself.

Troi facilitates a final reconciliation between Lwaxana and a manifestation of Kestra within her mind, allowing Lwaxana to finally let go of her guilt. Lwaxana awakens from her coma, and she and Deanna share a newfound understanding. Lwaxana is given a picture of Kestra and baby Deanna to help her remember that Kestra's life was filled with joy and to celebrate her life rather than mourn her death. The episode concludes with mother and daughter ready to move forward together, healed from the wounds of the past.

Captain Picard and Deanna question Maques, who struggles to articulate his observations about Lwaxana's mind. However, through a brief, intense telepathic burst directed at Deanna, he manages to convey the presence of 'bad thoughts' and the persistent 'dark place' within Lwaxana. Deanna interprets this 'dark place' as her mother's meta-conscious, a Betazoid psychological defense mechanism, possibly overwhelmed by a severe psychic trauma. Despite the lack of an identified traumatic event, Deanna remains hopeful for a physiological diagnosis from Beverly's ongoing sub-neural scan. Driven by a deep need to help her mother, Deanna decides to stay with Lwaxana and attempts telepathic contact herself. Act 3

Maques explains his attempt to help Lwaxana, revealing that her psyche has 'collapsed in on itself,' retreating into her meta-conscious mind. He offers to provide a telepathic 'bridge' that will allow Deanna to access the images and memories within Lwaxana's mind. Despite the inherent dangers and the uncertainty of success, Deanna, driven by the faint 'Help me...' she heard, convinces Captain Picard to allow her to proceed with this unprecedented mental intrusion. Upon entering Lwaxana's mind, Deanna finds herself in a dark, surreal Enterprise corridor, disorienting and echoing with the sound of dripping water. A menacing wolf growl and the recurring 'Help me...' voice establish a threatening and mysterious atmosphere. An illusion of Captain Picard appears, calmly ordering Deanna to sever the connection, but Deanna quickly discerns it as a construct of her mother's mind, speaking with Lwaxana's pained voice, attempting to push her away. Resisting this initial barrier, Deanna continues her quest, only to confront the physical manifestation of the menacing wolf, forcing her to flee. She then enters a room resembling her quarters but filled with child's toys and a serene view of Betazed, where she encounters an idealized manifestation of her deceased father. Data suggests that Hedril, like other figures appearing in a dream or a meta-conscious mind, could represent a vulnerable or repressed aspect of Lwaxana's psyche, reinforcing Picard's observation that everything Deanna encountered within Lwaxana's mind acted as a barrier designed to keep her out. With no physiological cause yet found for Lwaxana's coma, Dr. Beverly Crusher emphasizes the increasing likelihood that a traumatic event precipitated her condition. Deanna, frustrated by the lack of external evidence or any recent traumatic events, begins a meticulous search through Lwaxana's extensive personal logs and journals, hoping to uncover a hidden clue. This significant period begins approximately a year after Lwaxana's marriage and ends a few months after Deanna's own birth. Further investigation reveals that these files were not simply missing but were deliberately deleted by Lwaxana herself almost thirty years prior. This discovery provides the critical breakthrough, strongly suggesting a profound, repressed event occurred during that specific, erased period. Realizing the immense implications of this deliberate act of erasure, Deanna resolves that the only way to uncover the full truth and potentially save her mother is to re-enter Lwaxana's mind, convinced that the answers lie within this hidden, deleted past. Act 5

Deanna re-enters Lwaxana's mind, now armed with the knowledge of the deleted journal entries and determined to uncover the truth. Inside, 'Hedril,' now clearly identified as a Betazoid child and not Cairn, reappears with the menacing wolf. 'Hedril' poignantly questions her own identity, asking 'Who is Hedril?' before fleeing with the wolf, leading Deanna through a void into a dark, eerie arboretum. The persistent dripping sound is traced to the pond, where 'Hedril's' reflection briefly appears before being obscured. Deanna confronts Lwaxana, who stands guard over this final, most painful secret. Under Deanna's persistent questioning about the deleted journals and 'Hedril,' Lwaxana's emotional defenses finally crumble. She is forced to reveal a long-repressed memory: a family picnic at Lake El'nar with her husband, baby Deanna, and another daughter, Kestra, who was playing with a dog. The memory shifts to the tragic moment Kestra, chasing the dog, drowned in the strong current while Lwaxana was momentarily distracted with baby Deanna. Overwhelmed by unbearable guilt and grief, Lwaxana had repressed all memory of Kestra and meticulously deleted her from her journals. Deanna guides her mother through this agonizing revelation, urging her to forgive herself and to remember Kestra's life with joy rather than perpetual grief and self-blame. A manifestation of Kestra appears, and Lwaxana, finally able to express her profound sorrow and seek forgiveness, embraces her lost daughter, allowing Kestra's image to fade peacefully. Lwaxana awakens from her coma, healed and reconciled with her past.
